An organized next step
A general outline for coordinating a referral. Confirm referral arrangements directly with Dr. Goldstein.
Provide basic referral information through an agreed, appropriate channel.
Confirm patient contact preferences and discuss appointment availability.
Clinical findings guide recommendations; a referral does not guarantee treatment.
Where care is appropriate, follow-up and authorized communication depend on individual needs.
Patient history, symptoms, examination findings, functional limitations, available medical records and individual clinical needs guide recommendations.
Concerns may include neck pain, back pain, stiffness, reduced movement and activity limitations after injury. Headaches require appropriate assessment and are not automatically an indication for chiropractic care.
Suspected serious injury or significant neurological findings may require medical referral or co-management. Emergency symptoms require emergency medical care, not routine scheduling.
Clinical records
The medical record is first and foremost a clinical record. Depending on the encounter, it may document:
Not every record includes every element. No particular causation, impairment or permanency opinion is promised.
The goal is to keep referral coordination clear while keeping patient care central. Discuss administrative questions, record requests and clinically appropriate coordination with the office.
Care-related communication and records sharing are subject to appropriate patient authorization and applicable privacy requirements. Referral alone does not authorize automatic disclosure of medical information.
Some findings call for evaluation outside conservative musculoskeletal care. Significant neurological findings, suspected serious injury or other concerning symptoms may warrant additional medical evaluation.
Imaging or specialist assessment may be considered when clinically appropriate. Patient safety and appropriate scope take priority; this page is not an emergency referral protocol.
